To request a Return Authorization (RA) for your tape media complete the form below and click "Submit Form". - Only the media listed below are eligible for warranty replacement.
- No more than 20 cartridges can be returned at one time.
- If you need media troubleshooting assistance or have media returns in excess of 20 units, please call the customer support line.
- Serial numbers of all media must be provided.
- All media must have the Quantum logo printed on the case.

Note: Receiving a Return Authorization does not guarantee warranty replacement. The tapes must pass a physical inspection upon receipt. Physical damage, via carrier or otherwise, is not covered. Note: Ensuring customer confidentiality is a priority to Quantum. Whenever a tape cartridge is returned to Quantum you are insured that the data on the tape cartridge will be immediately erased using Quantum’s processes and equipment. A second step is taken to physically destroy the cartridge. With these steps Quantum eliminates any possibility of any unauthorized party retrieving previously recorded data. Returning your media: - After receiving a Media RA number send the media to the address found on the Return Instructions for Media page.
- You must use a traceable carrier (FedEx, UPS etc.).
- Ensure the RA number is included in the address and is clearly visible on the box.
